Transition at Three

The Transition, Birth to Three, Committee meets quarterly to discuss pertinent issues related to the transition process for children turning three who will be exiting the infant/toddler (birth to three) early intervention system and, if eligible, entering the pre-school early intervention system (Elwyn/SEEDS). Representatives to this committee consist of staff from service coordination and provider organizations, family members and others who are involved or interested in this transition process. Membership and meetings are open to all families, providers and other interested parties. Members of the committee meet regularly to discuss issues and procedures related to the transition process in an effort to continually improve transition between the two early intervention systems for families.
